All interested applicants must submit the following documents in one (1) complete set to the CHR-NCR Office:

a. Original application letter indicating the position you are applying for.
b. Duly accomplished and notarized Personal Data Sheet (PDS) with recent passport-sized picture (CS Form No. 212, Revised 2025) with Work Experience Sheet (WES), Forms and Guides to Filling out PDS can be downloaded from the Civil Service Commission Website (http://www.csc.gov.ph
).
c. Certificate of CSC Eligibility / IBP ID / PRC ID.
d. Photocopy of Transcript of Records.
e. Copy of Diploma
f. Photocopy of Performance rating for the last rating period (if applicable).
g. Photocopy of Training Certificates.

Please take note that the above list (‘a’ to ‘g’) is considered as one (1) set of required application documents. The set should be placed inside a long-sized folder, indicating the name of applicant (surname, first name, middle name) in each folder.

In addition to this, the applicant has to also submit three (3) original copies of PDS with WES.
