  2. b) Three (3) Original Copies and Duly accomplished and notarized Personal Data Sheet (PDS) kasama additional work experience sheet (WES), forms and Guide-to-Filling-Out the PDS can be downloaded at the Civil Service Commission (CSC) website https://web.csc.gov.ph/;
  3. c)   Kopya ng sertipiko ng CSC Eligibility/IBP ID/PRC ID;
  4. d) Certified true copy ng Transcript of Records (TOR);
  5. e)   Performance Rating para sa huling rating period (para sa mga aplikanteng nagtatrabaho sa gobyerno);
  6. f)   Kopya ng Diploma;
  7. g) Kopya ng mga sertipiko ng pagsasanay

 Kindly submit the required documents placed inside a long-sized folder, with the name of the applicant (Surname, First Name, Middle Name) clearly indicated on the folder.
